As a team-based medical home, we provide additional services to make sure that you and your family receive comprehensive and well-coordinated care.

Health insurance enrollment services are available to help you enroll under the Hawai‘i Med-QUEST (Medicaid) and other government benefits.  For more information, call (808) 872-4007.

Language access lines at all sites help translate for patients who speak languages that our staff does not speak.

Patient portal will give you online access to easily communicate with your provider, view appointments, test results, immunizations, medical summary, or send a message to your provider or care team.
